If you're going to learn one number before buying a nerve supplement, make it this one. Alpha-lipoic acid has the strongest research base in the category, and the dose used in that research gives you a benchmark to judge any label against.
Alpha-lipoic acid (ALA) is a compound your body produces in small amounts, involved in mitochondrial energy metabolism. It functions as an antioxidant and, unusually, works in both water-soluble and fat-soluble environments.
Oxidative stress is implicated in the nerve damage of diabetic neuropathy. ALA's antioxidant activity is the proposed mechanism — and unlike much of this category, it's been tested in humans rather than just in theory.
Human trials have examined ALA for diabetic neuropathy symptoms — burning, tingling, numbness and pain. The findings are genuinely encouraging for symptom scores, particularly in shorter trials.
Two honest caveats. First, effects on symptoms are clearer than effects on measured nerve function. Second, some of the strongest results come from intravenous administration in clinical settings, which isn't what a capsule delivers.
This is precisely why an unpublished panel is a problem. Without the number, the comparison is impossible.
ALA comes in two mirror-image forms. R-lipoic acid is the biologically active one; most cheaper supplements use a 50/50 racemic mixture. Some products list "R-ALA" specifically, usually at lower total amounts because it's more concentrated. Worth knowing if you're comparing products.
